This is a mandatory form.*

This form is to be completed and signed by both parties:

It must be filed by the parties at least 2 business days prior to the Trial (Paragraph 6.7 of the Case Management Directions). It sets out some basic facts for the assistance of the judge.

Number of copies of the document for filing, service and yourself
The completed and signed original of the document filed at the Court. Before you file it you must also make sufficient copies to have one for each person to be served and a copy for your own records.

Complete the form by typing (eg. on a computer or typewriter) or hand printed in ink.
