Annexure to Draft Consent Parenting Order

Instructions:
- Each party to the application for parenting orders or his/her lawyer must complete a separate annexure. Please refer to Consent Orders Supplement for use in parenting matters where proceedings have already been filed in the Family Court (Do it yourself kit).
- Staple the annexures to the back of the original of the proposed consent orders signed by the parties.
- Part B must be completed by the person who signs the annexure.
- Part C is only completed where there have been allegations of abuse.
