The Family Court and the Federal Magistrates Court are piloting a new Initiating Application form as part of the Combined Registry program. If successful, this form will replace Application in a Case for the Family Court.
You may use either the new Initiating Application or Application in a Case (see attachments below) to begin proceedings in the Family Court.

PLEASE NOTE: if using "Initiating Application" you MUST use "Response to Initiating Application" in response. You cannot use a combination of pilot forms and Application in a Case and Response to an Application in a Case.

This form is used:
Making copies of the form for filing, service and yourself
The completed and signed original of the form is filed at the Court. Before you file it you must also make sufficient copies to have one for each person to be served and a copy for your own records

An affidavit unless the application is being used to review the decision of a judicial registrar, registrar or deputy registrar.
Who you serve
Each party to the case, ie. those named as respondent/s on the application, and any independent children's lawyer
