All forms used in the Family Court and Family Law Courts in alphabetical order
The Family Court and the Federal Magistrates Court are piloting a new ‘Initiating Application’ form as part of the Combined Registry program. If successful, this form will replace ‘Application for Final Orders’ for the Family Court.
You may use either the new ‘Initiating Application’ or ‘Application for Final Orders’ (see attachments below) to begin proceedings in the Family Court.

PLEASE NOTE: if using ‘Initiating Application’ you MUST use ‘Response to Initiating Application’ in response. You cannot use a combination of pilot forms and Application for Final Orders and Response to an Application for Final Orders.

Application for Final Orders
This is a mandatory form.*
This is the Court's main application form. It is used by a person who wishes to apply for final Court orders, other than a divorce, for example, property, parenting, child support, maintenance.
Making copies of the form for filing, service and yourself
The completed and signed original of the form is filed at the Court. Before you file it you must also make sufficient copies to have one for each person to be served and a copy for your own records.

Who you serve
Each party to the case, ie. those named as respondent/s on the application.
Note: You may be required to serve other people or organisations with a copy of the application, for example, if a child support application, the Child Support Agency.
