Bankruptcy - Notice to creditors of annulment application

This form is used by the applicant for annulment of a bankruptcy to give notice of that application to creditors.

Making copies of the form for filing, service and yourself
The completed and signed original of the form is filed at the Court. Before you file it you must also make sufficient copies to have one for each person to be served and a copy for your own records

Who you serve
You must serve the notice on each person known to you to be a creditor of the bankrupt at least 14 days before the hearing date fixed for the application.
