This is a mandatory form.*

This form must be filed by a party to a financial case at least 7 days prior to a conciliation conference. See Rule 12.05(2)(a) of the Family Law Rules 2004. It sets out the parties current financial circumstances, a brief summary of the facts relied upon by a party in seeking the orders sought, and the party's proposal for settlement.

Number of copies of the document for filing, service and yourself
The completed and signed original of the document filed at the Court. Before you file it you must also make sufficient copies to have one for each person to be served and a copy for your own records.
