This form is to be used if you are entitled to intervene under the Family Law Act or other legislative provision, such as the Judiciary Act (Attorney-General) or child support legislation (Child Support Registrar).
This form is not to be used by a person who wishes to become a party to a case. In this circumstance you must file an Application in a Case (see Rule 6.05).
Making copies of the form for filing, service and yourself
The completed and signed original of the form is filed at the Court. Before you file it you must also make sufficient copies to have one for each person to be served and a copy for your own records.
