A list of forms beginning with the letters N through Z
This form has been prepared by the Court to help a person notify the Court that they would like a new named used on all future Court documents. Refer to Rule 24.03.
Number of copies of the notice for filing, service and yourself
The completed and signed original of the notice filed at the Court. Before you file it you must also make sufficient copies to have one for each person to be served and a copy for your own records.

In completing a Court form, you must
Complete the form by typing (eg. on a computer or typewriter) or hand printed in ink.
